All applicants using the DS-11 application form must appear in person to process their application for passport. Parental consent of both parents is required for issuance of a passport to a minor under age 14.
Passports are received within 6 weeks of the date of application.
Expedite service is available for an additional fee of $60.00 if you need your passport returned within 2 weeks. If you have a more urgent departure within 14 calendar days, an appointment is required at the Connecticut Passport Agency, located at:

Passport fees for routine service, using DS-11 form:
10-year Passport - Age 16 & over $55.00 + $30.00 execution = $85.00
5-year Passport - Under age 16 $40.00 + $30.00 execution = $70.00
